If you are planning on taking your child out of school during term time, please complete the below form and return to the school office, wherever possible in excess of 4 weeks prior to the leave date. This will be considered by the head teacher and a letter will be sent out to you. Only in exceptional circumstances may term time leave be authorised as holidays should be taken within the 13 weeks school holidays.
